About The Program

Open Disclosure is the open discussion of incidents that result in harm to a patient while receiving health care. The essential elements of open disclosure are outlined in the National Open Disclosure Standard (the standard).

The objective of the Commission's Open Disclosure program is to support jurisdictions and facilities to implement the standard, by undertaking the activities which are essential to the implementation of the standard and which can most effectively be undertaken at the national level.

Open Disclosure Standard

Open Disclosure is the open discussion of incidents that result in harm to a patient while receiving health care. The essential elements of open disclosure are outlined in The National Open Disclosure Standard (PDF 495 KB).

In April 2008, Health Ministers agreed to work towards the implementation of the National Open Disclosure Standard in all health care facilities.

Supporting implementation of the Open Disclosure Standard

The Commission’s Open Disclosure Program includes a number of initiatives to support jurisdictions and facilities to implement the Open Disclosure Standard; all aim to ensure that Open Disclosure is undertaken in ways that are most effective for patients. The initiatives include:
